ceremony package in "Description"
box. We track & file accounts by Date, so always put date when
writing us.

- Enter dollar amount to be
charged to your card. Please round off to whole dollars, no
cents. (2007 tax is 4.7%)

Hint: On the form's
bottom right column, check the box "Shipping
Info is the same as Billing" unless you are a 3rd party paying
for a couple's reservation, then enter their names above,

Remember to go back to the
booking form and click "submit" button
if you did not do so yet.
When you click Submit on the Booking
Form, your entries will be sent
to us automatically. If you did not click Submit, we will not
get your order. When we receive your payment, we acknowledge your Deposit, or Payment,
then we
confirm your requested date, time, location and professional services ordered.
To pay by mail, please make check/money
order to Rev. Kermit &
send at least 3 weeks in advance of your
date.
